眼窩周辺疾患への内視鏡下鼻内手術の応用

Endoscopic Endonasal Surgery for Ophthalmopathy

　緒言

　近年，慢性副鼻腔炎の外科的治療法として内視鏡を用いた鼻内経由の副鼻腔炎手術法が確立した。内視鏡の導入により鼻腔および副鼻腔の微細な観察が可能となり，種々の鼻副鼻腔疾患においても応用されるようになった。本術式を鼻副鼻腔に関連した眼窩周辺の疾患に適応できることが確認されたので報告する。

We reported the clinical application of endoscopic endonasal approach to five types of surgery related to ophthalmopathy; i) orbital decompression, ii) decompression of the optic nerve, iii) recon-struction of blowout fracture of the medial orbital wall, iv) drainage of subperiosteal abscess of the orbit, and v) dacryocystorhinostomy. Although the technique with an endonasal endoscope should be performed by surgeons with extensive experi-ence, all cases resulted in safe and accurate proce-dures without intraoperative and postoperative complications. These surgical procedures have the potential to reduce surgical severity, complication, and hospitalization.
