A 48-year-old female with dysthyroid optic neur-opathy received a decompression of the medial orbital wall using an endoscopic intranasal approach. Her visual acuity was deteriorated to 0.6 in the right eye and 0.2 in the left eye even after a high-dose corticosteroid treatment. Color visionwas deteriorated and visual-field was severely lim-ited. However, extra-ocular movements were minimally restricted and Herter measurements were minimally increased. Since she had received a Cal-dwell-Luc operation, the medial bony wall of the orbit was relatively thickened and was removed by an angled currette and a chisel. Orbital decompres-sion resulted in a remarkable improvement of visual acuity, color vision and visual-field. It was conclud-ed that the endoscopic transnasal approach for orbital decompression provides satisfactory results.
