内視鏡下経鼻的副鼻腔手術による副鼻腔真菌症の治療

Endoscopic Endonasal Sinus Surgery for Paranasal Sinus Mycosis

　はじめに

　近年，鼻副鼻腔真菌症の報告例は増加の傾向にあり，ステロイド剤の多用や抗生剤の乱用に伴う菌交代現象などがその一因ではないかといわれている。CT，MRIの利用により鼻副鼻腔真菌症の診断は向上しているが，その臨床所見は悪性腫瘍とまぎらわしいためその鑑別が重要である。治療に関しては真菌症を含め副鼻腔炎に対しては病的粘膜を完全除去する根本手術が一般的であったが，光学技術の発達とともに鼻副鼻腔手術に内視鏡が応用され，鼻内経由の内視鏡下の操作で各副鼻腔を単洞化し粘膜を温存し治癒せしめる術式が行われるようになってきている^1）。副鼻腔真菌症自験例7例に，内視鏡下で鼻内経由に上顎洞自然口を開大し粘膜保存する手術を施行し良好な結果を得た。これらの症例の背景因子，画像診断，病理組織，手術所見と結果について報告する。

Seven cases of paranasal sinus mycosis, three males and four females, were reported. In diagnosis of paranasal sinus mycosis, fungal culture test was negative in all cases, however, CT and MRI findings were very valuable and characteristic to mycotic sinusitis. Six patients showed a calcification in the maxillary sinus. A low signal in both T1 and T2 of MRI was recognized in six patients. Endoscopic endonasal sinus surgery with preservation of sinus mucosa was performed in all patients with maxillar-y mycosis. Symptoms disappeared immediately after surgery. The sinus cavity mucosa showed normal appearance several months later, suggesting that mucociliary function of sinus mucosa was recovered and normalized after surgery.
