上顎洞根治手術での同種乾燥硬膜の使用について

Use of Lyophilized Dura in the Caldwell-Luc Operation

　はじめに

　副鼻腔炎の術後数年から10数年，さらには数10年を経て発生する術後性上顎嚢腫や術後の再発生副鼻腔炎の数には少なからざるものがある。そして上顎洞再手術で手術を難しくしていることの一つに，欠損している上顎洞骨前壁の部分から洞内に入り込んでいる瘢痕組織がある。

　手術は元の形態を保持するのが理想であり，この点からもosteoplastyや骨膜縫合などの工夫が試みられてきた。osteoplastyは骨弁を採取することが必ずしも容易でない。特に前壁骨が薄いと，採取のさい割れやすく，また骨弁端の整復が思うようにならない。骨膜縫合はその点では容易であるが，確実に骨膜縫合ができているという自信のない場合もある。再手術ともなると，結合織の洞内侵入があり，それは不可能となる。

　これらのことから欠損した上顎洞前壁を補完し，結合織侵入を防ぐために同種乾燥硬膜（lyophi—lizcd homograft dura，以下，LHDと略）を使用してみたが，ほぼ目的に適合してつることが分かってきたのでここに報告す。

There are a quite few patients who were ope-rated on for removal of cyst within the maxillary antrum developed several to ten or more years after the first sinus operation. The second opera-tion is difficult and troublesome owing to scar tissue that had entered into the antrum cavity from buccal and labial soft tissue through the opening in the anterior bony wall of the antrum made during the previous operation. In order to prevent invading scar tissue into the maxillary cavity and deformation of the maxillary sinus, the opening of the anterior bony wall was covered with a piece of lyophilized homograft dura (LHD). Sixty-eight cases were treated by this method, and the satisfactory results were obtained.
