原発性孤立性蝶形骨洞真菌症の1症例

Primary and Isolated Fungal Infection of the Sphenoid Sinus

　はじめに

　副鼻腔真菌症の中で蝶形骨洞真菌症の報告例は少ない。蝶形骨洞以外の副鼻腔や眼窩内にも真菌感染が認められるものや，また免疫能低下や経蝶形骨洞下垂体手術後，外傷後などで頭蓋内にも真菌感染が認められるような症例は散見されるが，基礎疾患がなく蝶形骨洞のみに単独に原発する真菌感染は文献上さらに数少ない。筆者らは最近これに相当する症例を経験したので報告する。また本症例ではX線透視下に鼻内蝶形骨洞開放術を施行したが，その手術経験も併せて報告する。

A 48-year-old female presented with a 5-day-history of severe headache and right retro-orbital pain. Computed tomography depicted soft tissue density of the right posterior ethmoid sinus and the right sphenoid sinus. Intranasal transethmoidal sphenoid surgery was performed under a x-ray TV monitor. Histological examination revealed that the surgical specimen from the sphenoid sinus was a fungus ball of Aspergillus species. X-ray TV monitor proved to be useful for intranasal surgery.
