はじめに

　喉頭蓋囊胞は日常臨床においてしばしば遭遇する疾患であり，臨床的には無症状であることが多く，また良性疾患であるため経過観察されやすい。しかしながら，時に嚥下時の違和感，咽喉頭異常感，気道閉塞などの呼吸症状を呈することがあり，有症状の場合は囊胞摘出術が行われる。今回われわれは，喉頭蓋谷に存在した囊胞状腫瘤が3年の経過で喉頭蓋舌面に基部をもち，長い茎を有する振子様の状態に変化した1例を経験したので報告する。

　We report the case of a 59-year-old man with an epiglottic cyst that developed a pendulum-like appearance with a long stalk during follow-up. At 56 years of age, the epiglottic cyst was asymptomatic and fixed in the epiglottic vallecula. At the age of 59 years, the patient was referred to our department for a pendulous cystic mass identified on upper gastrointestinal endoscopy. The epiglottic cyst was positioned in the postcricoid area, pedunculated from the right side of the epiglottis, and showed mobility while coughing and swallowing. Since the patient had severe throat discomfort, laryngomicrosurgery under general anesthesia was performed. Histopathological examination revealed a tonsillar cyst.
