動画を用いた耳鼻咽喉科医以外のための鼻出血対応マニュアル

A manual for non-otolaryngologists to treat nosebleeds

はじめに

　鼻出血は耳鼻咽喉科救急疾患のなかでも頻度が高く，救急外来を受診した耳鼻咽喉科患者の約8〜10％^1,2）という報告もある。時間外に受診した場合，その対応は当直医が行うことになるが，慣れていない医師が救急搬送要請を断ることや，止血に難渋し耳鼻咽喉科コールとなることも珍しくない。さらに，実際に耳鼻咽喉科医到着時は止血しており処置の必要がない場合や，ガーゼ挿入の際に鼻粘膜を傷つけている場合もある。結果として対応時間が長くなり，患者，当直医，耳鼻咽喉科医それぞれの負担となる。そこで当科では，耳鼻咽喉科医以外のための鼻出血対応マニュアルを動画付きで作成し，院内で周知した。今回われわれはその内容を紹介するとともに，その効果についても検討したため，若干の文献的考察を踏まえて報告する。

　In emergency rooms, non-otolaryngologists are required to examine patients with nosebleeds on several occasions. However, many doctors aren't familiar with the requisite procedures to treat nosebleeds. Thus, they refuse emergency transport requests for patients with nosebleeds and call on otolaryngologists to treat the difficulty in hemostasis. These become a burden on patients, non-otolaryngologists, and otolaryngologists. Therefore, we made a nosebleed manual for non-otolaryngologists and circulated it among healthcare workers in our hospital. Since then, the frequency of calling on otolaryngologists and the refusal of emergency transport requests for such patients has decreased. The results of the questionnaire indicated that the manual was useful.
