直達喉頭鏡下に摘出した巨大喉頭蓋囊胞の1症例

A Large Epiglottic Cyst Removed Under Direct Laryngoscopy

I．はじめに

　喉頭蓋囊胞は日常臨床でよくみかける疾患であるが，咽喉頭違和感や嚥下障害をきたし，さらに大きさが増すと呼吸障害などの重篤な症状を惹起する疾患である。喉頭蓋舌面から発生することが多く，組織学的には類表皮囊胞や貯留囊胞が多い^1～3）。

　今回われわれは，喉頭蓋谷から喉頭蓋舌面にかけて基部をもつ巨大な喉頭蓋囊胞を直達喉頭鏡下に摘出し，病理学的に類表皮囊胞であった症例を経験したので報告する。

　A 66-year-old woman was complaining of abnormal sensation of the throat. Fiberscopic laryngoscopy revealed an epiglottic cyst measuring 3cm in diameter. Scopic and image findings were compatible with epidermoid cyst. The cyst was removed under general anesthesia using a direct laryngoscope. Histopathological examination revealed epidermoid cyst.
