当科における鼻出血353例の検討

Clinical study of 353 epistaxis cases

はじめに

　鼻出血は，耳鼻咽喉科の日常診療や救急現場でよく遭遇する一般的な疾患の1つだが，出血部位の同定が難しい場合や出血部位によっては，しばしば止血に難渋し，入院対応や止血手術が必要となる。当科では過去に，2009〜2013年の5年間の鼻出血489例の臨床的検討^1）を行った。今回われわれはさらにその後2014〜2018年の5年間の鼻出血症例の臨床経過をまとめ，鼻出血のリスク因子，特に難治例（入院例・再出血例・手術例）の傾向を検討したので報告する。

　We examined 353 cases of epistaxis patients who visited our hospital between January 2014 and December 2018.

　The results confirmed that there were certain risk factors for epistaxis：over fifty years old, male, winter, hypertension, smoking, and taking antithrombotic drugs.

　Additionally, in contrast with young cases, in which almost every epistaxis is from an anterior area, in senior cases, more bleeding is observed from the other areas.

　In order to select an appropriate treatment, it is critical to estimate the site of nasal bleeding by considering the background of each patient.
