喉頭微細手術下に摘出した内側型Laryngoceleの1例

A case of internal type laryngocele treated with laryngomicrosurgery

はじめに

　Laryngoceleは本邦においては稀な疾患で，喉頭室より上方に伸びた喉頭小囊が空気を含み囊状に拡大した疾患である。気囊胞の占拠部位により，①喉頭内にとどまる内側型，②側頸部に限局する外側型，③喉頭内から側頸部へ連続する混合型，の3型に分類される^1）。今回われわれは内側型laryngoceleの1例を経験したので報告する。

　A 42-year-old man presented with the chief complaint of the vocal disorder. Laryngoscopic examination revealed a swelling of the right false vocal fold. CT and MRI showed an air-filled mass located in the right paraglottic space. The mass was totally resected by laryngomicrosurgery with direct endoscope using CO₂ laser under general anesthesia. Pathological finding was compatible with the laryngocele. The clinical course was uneventful after surgery. Resection of the internal type laryngocele by laryngomicrosurgery appears to be a low invasional and acceptable mode of treatment.
