頸部外切開により摘出した巨大な口腔底奇形腫様囊胞の1例

A case of giant teratoid cyst in the floor of the mouth totally resected by a transcutaneous approach

はじめに

　類皮囊胞は身体各部に発生するが，頭頸部領域におけるその発生頻度は6.9％，口腔付近に発生する頻度は1.6％と比較的稀である^1）。

　類皮囊胞は，病理組織学的に（狭義の）類皮囊胞，類表皮囊胞，奇形腫様囊胞の3種類に分類され，なかでも奇形腫様囊胞は非常に稀である。

　今回われわれは，口腔底に生じた巨大囊胞性病変を頸部外切開により摘出し，奇形腫様囊胞と診断した1例を経験した。本症例の画像所見や病理組織所見，手術方法について文献的考察をつけ報告する。

　A 59-year-old man was admitted to our hospital with the chief complaint of the neck swelling and dysarthria. CT and MRI showed a cysitic mass in the floor of mouth and entrapped by mylohyoid muscle.

　By transcutaneous approach the cyst was totally resected. Pathological findings, showed teratoid cyst.

　Teratoid cyst is a variant of dermoid cyst which rarely occurs in the floor of mouth.
