　白内障手術後にメチシリン耐性黄色ぶどう球菌（MRSA）による眼内炎症を起こした1症例を経験した。症例は89歳の女性で慢性涙嚢炎，糖尿病を合併していた。手術待機中に右眼に膨化白内障を認めたため，緊急に計画的嚢外摘出術と眼内レンズ移植術を行った。術後3日目に術創の角膜浸潤と前房蓄膿，豚脂様角膜裏面沈着物を伴う急性炎症を発症した。ただちに前房洗浄を行い，同時に採取した前房水を培養した結果，多剤耐性のMRSAが検出された。感受性のあるミノサイクリンの全身投与で炎症は改善した。この症例においてMRSAは，涙嚢から結膜を通じ，眼内感染を起こしたものと考えられた。

A 89-year-old female underwent emergency sur-gery for intumescent cataract in her right eye. Shehad been suffering from diabetes mellitus andchronic dacryocystitis. We performed plannedextracapsular cataract extraction with a three-piece intraocular lens implantation. On day 3 aftersurgery, acute intraocular inflammation evelopedwith cell infiltration along the corneoscleral woundaccompanied by hypopyon and mutton-fat keraticprecipitates. Anterior chamber was tapped immedi-ately and irrigated with antibiotic solution. Cultureof the aqueous showed methicillin-resistant Sta-phylococcus aureus (MRSA) which was sensitive tominocycline. Treatment with systemic minocyclineeffectively controlled the inflammation. The detect-ed MRSA would have been present in the infectedlacrimal sac and penetrated into the corneoscleralwound after cataract surgery.
