メチシリン耐性黄色ぶどう球菌による術後眼内炎の1例

A case of methicillin-resistant Staphylococcus aureus endophthalmitis after cataract surgery

　90歳の男性。水晶体嚢外摘出術と人工水晶体挿入術後に，メチシリン耐性黄色ぶどう球菌（MRSA）による眼内炎が生じた。術後4日目に豚脂様角膜後面沈着と前房内のフィブリン反応が生じ，6日目には，角膜輪部に手術創につながる角膜浸潤巣や前房蓄膿が出現した。眼痛や眼瞼結膜浮腫はなく，前眼部の刺激症状は乏しかった。細菌性眼内炎が疑われ，前房洗浄と硝子体切除，部分的後嚢切除を行った。術中得られた角膜後面白色塊状物と前房中のフィブリン塊からMRSAが検出された。感染経路は不明であった。イミペネムとミノサイクリンの2剤併用を行い，眼内炎は消退した。

A 90-year-old male underwent planned extracap-sular cataract extraction with intraocular lens im-plantation in his right eye. Fibrin reaction with muttonfat keratic precipitates appeared 4 days after surgery. Hypopyon and infiltration along the corneoscleral incision developed 2 days later. Ocu-lar pain or chemosis was absent. The findings were suggestive of bacterial endophthalmitis. The eye was treated by anterior chamber irrigation.vitrectomy and partial capsulectomy. Methicillin-resistant Staphylococcus aureus was isolated from the anterior chamber aspirate. Clinical cure ensued following additional systemic minocycline and imipenem. It was not possible to identify the route of infection.
