A 90-year-old male underwent planned extracap-sular cataract extraction with intraocular lens im-plantation in his right eye. Fibrin reaction with muttonfat keratic precipitates appeared 4 days after surgery. Hypopyon and infiltration along the corneoscleral incision developed 2 days later. Ocu-lar pain or chemosis was absent. The findings were suggestive of bacterial endophthalmitis. The eye was treated by anterior chamber irrigation.vitrectomy and partial capsulectomy. Methicillin-resistant Staphylococcus aureus was isolated from the anterior chamber aspirate. Clinical cure ensued following additional systemic minocycline and imipenem. It was not possible to identify the route of infection.
