　閉塞隅角緑内障（ACG）をはじめとする緑内障眼に伴う水晶体混濁の実態，機序については，いまだ不明瞭な点が多いが，眼圧上昇機序を明らかにする上でも重要である。今回，筆者らは、混濁の種類，その進行のパターンなどの臨床的所見と，白内障手術時に得られた水晶体上皮の組織所見との相関についての検討を行った。対象は，白内障のために水晶体嚢外摘出術を行った184眼である。緑内障種類別内訳は，白内障手術以前に観血的手術既往のない，ACG20眼，POAG30眼，PEG11眼，さらに，緑内障手術既往のある各種緑内障眼（OPE）23眼。同年齢層の緑内障を伴わない白内障眼100眼を対照とした。水晶体上皮の組織的検討においては，術時に得られた前嚢を用いて上皮細胞の密度，面積，さらに前嚢の厚さについて行った。

　その結果，水晶体混濁の種類はACG群に核白内障の率が有意に高く，水晶体上皮細胞に関しては，緑内障群，OPE群ともに対照群と比べて面積増大，密度減少が認められた。また，水晶体嚢の厚さについて，今回の検討は，前嚢の中央でのみの検討であるが，ACG群は，対照群に比べて有意に厚いという結果が得られた。

We evaluated the lens opacity in 184 eyes with glaucoma. The series comprised primary angle closure glaucoma 20 eyes, primary open angle glaucoma 30 eyes, pseudoexfoliation glaucoma 11 eyes, and 23 eyes with past history of glaucoma surgery. Another series of 100 nonglaucomatous eyes served as control. Each eye was evaluated as to the clinical feature of cataract, size of lens epithelial cells and thickness of the anterior capsule in specimen obtained during cataract surgery.

There was a high incidence of nuclear cataract in primary angle closure glaucoma. All the 4 groups with glaucoma manifested increased cell size in the anterior lens capsule and decreased cell density as compared with nonglaucomatous eyes. Eyes with primary angle closure glaucoma manifested thicker anterior lens capsule than in nonglaucomatous eyes.
