Pseudo-Foster Kennedy症候群をきたした前部虚血性視神経症の1症例

A case of bilateral anterior ischemic optic neuro-pathy masquerading Foster-Kennedy syndrome

　両眼発症の前部虚血性視神経症（AION）によるpseudo-Foster Kennedy症候群の1症例を経験したので報告した。症例は73歳男性で，主訴は両眼の視力，視野障害であった。全身的には高血圧，糖尿病があり，眼底には右視神経萎縮，左うっ血乳頭を認めた。Foster Kennedy症候群を疑い，脳神経学的検査を施行したが，頭蓋内に異常は認められなかった。螢光眼底検査で，両眼の視神経乳頭に扇状の充盈欠損を認めた。高齢者でFoster Kennedy症候群様の検眼鏡所見がみられた際には，本症によるpseudo-Foster Ken-nedy症候群を十分考慮する必要があり，診断には螢光眼底検査が有用であると考えられた。

A 73-year-old male presented with progressive impairment in vision and visual field in both eyes. Systemic hypertension and diabetes mellitus were present as associated findings.

We observed optic atrophy in his right eye and disc swelling in the left. In spite of the clinical picture of Foster-Kennedy syndrome, neurological examinations failed to reveal intracranial mass lesions. Fluorescein angiography showed sector -shaped dye-filling defect in the optic disc in both eyes. We diagnosed the case as bilateral ischemic optic neuropathy.
