白内障手術における術中硝子体圧上昇の危険因子

Risk factors for increased vitreous pressure during cataract surgery

　硝子体圧の上昇をきたす術前の全身的因子について検討し，その防止策について考察した。対象は白内障手術を施行した症例のうち術中に前房消失ならびに虹彩脱出を認めたため，高張浸透圧剤の点滴をした症例である。その結果硝子体圧上昇を認めた症例は1,116眼中45眼（4.0％）であった。危険因子は高血圧症，術中の血圧上昇，肥満女性および近視眼であった。術中に硝子体圧の上昇が予測される症例には術前，術中の血圧コントロールなどの全身管理，眼球マッサージや完全な麻酔による眼局所管理が必要である。

In a consecutive series of 1,116 cataract surgeries,collapse of anterior chamber or iris prolapse occur-red in 45 instances, 4.0%. As risk factors for in-creased vitreous pressure, we identified systemichypertension, acute rise in blood pressure duringsurgery, obesity in females and myopia. In thesehigh-risk cases, we advocate more strict control ofblood pressure before and after surgery, eye mas-sage and deep topical anesthesia.
