A 75-year-old female with high myopia and posterior staphyloma underwent uneventful extracapsular cataract extraction in her left eye. The visual acuity improved to 0.7. One year later, the visual acuity deteriorated to 0.3 due to after -cataract. We performed Q-switched YAG laser capsulotomy with 14 bursts at 2 mJ. A pupil hole of 4 mm in diameter resulted and the visual acuity improved to 0.7.

One month later, retinal detachment developed with a macular hole and peripheral retinal hole. The vitreous was anteriorly detached and herniated into the anterior chamber. The retina could be reattached after vitrectomy and scleral encircling procedure.

This case illustrates the importance of fundu-scopic evaluation following YAG laser cap-sulotomy. It is advocated to keep the capsulotomy hole to a minimal size in high-risk cases.
