We performed cataract extraction with implanta-tion of posterior chamber intraocular lens (IOL) in 6 eyes with small pupil. Glaucoma was present in 5 eyes and 2 eyes underwent prior filtrating surgery. The maximum pupil size measured 2 to 4.5- mm in the series.The iris was atrophic in all the cases.

To facilitate delivery of the lens nucleus and ensuing irrigation-aspiration,we performed basal iridotomy of about 4 mm in length.The IOL was inserted through the pupil in 3 eyes and through the iridotomy opening in the other 3 eyes.Thereafter, the iridotomy was repaired by tying pre-placed sutures. This procedure could prevent possible damages to the pupil during nucleus extraction and cortex evacuation procedures. The IOL could be implanted with more ease in the bag when placed through the iridotomy opening. As a further merit, it was pos-sible to maintain the round pupil in the center of the iris area.
