慢性緑内障眼における羞明に関する検討

Glare disability in chronic glaucoma

　視神経疾患では，高輝度下における視力低下，いわゆる羞明を訴えるとされている。近年，緑内障において初期より中心視機能障害が存在する事が示唆されている。我々は，中期までの緑内障眼における高輝度下での視機能障害を検討した。

　対象は，前眼部と中間透光体に混濁のない，年齢15〜58歳，矯正視力1.0以上，視野湖崎分類Ⅰb期〜Ⅲb期までの慢性緑内障眼41例70眼である。グレアテスターによる検査を施行し，グレア難視2.5％の視標が見えるか見えないかの2群に分類し，視野ステージと，オクトパス自動視野計による傍中心6度での感度低下との相関を調べた。

　その結果，グレア視力障害は傍中心での緑内障暗点と相関していた。緑内障初期の段階からグレア視力に影響を及ぼす中心部視神経障害の存在が示唆された。グレアテスターは緑内障初期暗点診断に有用である。

We evaluated 70 eyes with chronic glaucoma inearly to middle stages of the disease. We paidparticular attention to photopsia and photophobiaand employed the Millar-Nadler glare tester. Glaredisability under 2.5% disability was found in 67% ofthe eyes. The disability was well correlated withthe central visual field damage defined as central 8-point damage by Octopus automated perimetry.The findings show that glare disability sets in fromthe early stage of chronic glaucoma and that it isprobably related with foveal dysfunction in thisdisease.
