We evaluated the changes in epithelial cells in cataractous lens in relation to age, type of cataract, presence of diabetes mellitus and glaucoma. We obtained, by extracapsular cataract surgery, 83 specimens from otherwise normal eyes, 18 from diabetic persons and 8 with a history of acute glaucomatous attack.

The epithelium adhering to the anterior capsule was fixed immediately in formalin and stained with toluidine blue. The central part of the specimen wasphotographed under a light microscope. We then measured the size and density of the cells by an automated measuring system for the corneal en-dothelium seen by specular microscopy.

The cell size and density were age-dependent. There was a significant decrease in cell density in mature cataract in otherwise normal eyes. The lens epithelium in glaucomatous eyes showed signifi-cantly different features in cell size and density. The findings seemed to indicate that the damage to the lens epithelium by acutely elevated intraocular pressure is different in nature and extent from that due to aging.
