緑内障視野障害進行に伴う乳頭陥凹と色調変化との相関

Correlation between cupping and color of glaucomatous optic disc with progressive visual field defect

　視野障害進行のある緑内障症例20眼に対し，独自に開発したカラー画像解析システムを用いて乳頭色調の変化を客観的かつ定量的に捉え，陥凹の変化との相関につき検討した。ごく初期でも陥凹拡大に伴い色調変化がおこることが示され，視野障害の進行を認めた症例ではごく初期においても陥凹拡大に伴いその部の乳頭辺縁部の色調が変化することが示唆された。視野障害がある程度進行した例では，陥凹拡大を示さずに色調変化を生じる例もみられ，視野障害の進行が高度となり，乳頭陥凹がある程度拡大すれば陥凹拡大を示さずに色調のみが変化すると考えられた。

We developed a color image analysis system to quantitate the color of the photographed optic disc. This system was applied to 20 eyes with primary open angle glaucoma with progressive visual field defect. In 2 out of 7 eyes in early-stage glaucoma,the disc rim color changed along with the enlarge-ment of the cupping. A similar finding was obser-ved in 4 out of 11 eyes in middle-stage glaucoma. The disc rim color changed while the cupping remained stationary in an eye with progressed visual field defect.

The findings suggested that the color of the optic disc may change in early-and middle-stage glau-coma with associated enlargement of disc cupping.
