緑内障半視野テストによる視野障害評価の信頼性

Reliability of glaucoma hemifield test

　緑内障眼および緑内障疑いの症例92例172眼に対しハンフリー自動視野計STATPAC2の緑内障半視野テストの臨床評価を行った。上下いずれかの半視野に限局した局在性の閾値低下には，多くの症例に正常範囲外の判定を下したが，上下の半視野に同程度の閾値低下を認める症例では判定が困難な場合があった。これらの症例は，屈折，前眼部，中間透光体，瞳孔径に異常を示さず，初期の緑内障に認められる全般的な感度低下と考えられ，半視野テストの判定において注意が必要と考えられた。

We evaluated the reliability of glaucoma hemifield test of Humphrey Field Analyzer®. We measured the visual field thresholds by program central 30-2 in 172 eyes of 92 subjects with definite or presumed glaucoma. Localized sensitivity loss was correctly diagnosed when the sensitivity losswas localized in the upper or lower hemifield. But in some cases without abnormalities in refraction lens, media or pupil size, were diagnosed “within normal limits” in presense of glaucomatous optic disc change and sensitivity loss.
