緑内障視野進行に伴う乳頭変化の検討—第I報初期緑内障例

Correlation between glaucomatous optic disc and visual field defects.:I. Early glaucomatous stage

　緑内障早期の視野障害の進行と乳頭変化との相関について検討を行った。対象は初期原発性開放隅角緑内障で，（1）同一眼で平均8年1ヵ月に及ぶ追跡中に初期視野障害の発現または進展を認めた4例4眼，（2）同一症例で視野障害の程度に左右益を認める24例48眼である。

　視野障害と陥凹変化との相関の検討にあたっては，神経線維の走向を考慮し，乳頭ならびに視野を各々上下に2分割して検討した。陥凹変化の検討にあたっては，陥凹乳頭面積率（C／D・A）を用いた。

　その結果，Goldmann視野障害検出に先行して，陥凹の拡大が生じる時期があり，視野障害検出までに，約20％のC／D・Aの増大を要し，陥凹の拡大は，全方向に均等に起こる事を示した。

　これらは，神経線維がび漫性に脱落していく時期である可能性が考えられた。

We evaluated the correlation between glaucoma-tous optic disc and early visual field defects. The cup/disc area ratio was used as one of the parame-ters. Our study was made on 4 eyes (group I) who could were followed up for an average of 8.1 years and on 48 eyes of 24 cases (group II) who manifest-ed different degrees of visual field defect between both eyes. In the analysis of relationship between the disc and visual field, the disc and visual field were evaluated for superior and inferior halves respectively. This procedure seemed to conform to the distributional pattern of the nerve fiber in the optic disc and the retina.
