緑内障における乳頭陥凹とGoldmann視野

Quantitated glaucomatous disc cupping and Goldmann visual field

　縦の格子縞に角度をつけて眼底に投影し，眼底に写った各格子縞の位相差から眼底を定量立体計測できる筆者らの装置を使い，乳頭陥凹を計測した。なお陥凹の深さは等高線で表示した。

　正常眼乳頭でも150μ以上の陥凹は存在したが比較的乳頭中央部であった。緑内障眼ではGold-mann視野に変化がなくても等高線が多くなる—すなわち陥凹が深くなっていた。視野に変化のある乳頭は，視野障害に相当している乳頭陥凹の深い部分にある等高線が拡大していた。

　これから乳頭陥凹定量、71体計測によりGold-mann視野変化を推測することができる。

We quantitated the optic disc cupping in two glaucoma patients. By obliquely projecting grated lines onto the fundus, we could express the contour of the cupping as sets of isopters for every 120μm in depth. The glaucoma eyes were characterized by more numerous isopters for the cupping than nor-mal eyes, suggesting deeper and wider excavation. In 2 eyes with glaucomatous visual field defect, the defect by Goldmann perimetry coincided with the location of the deepest area of the thus expressed disc cupping.
