62歳の女性が11年前に右眼打撲で受診した。矯正視力は右光覚弁，左0.3で，＋7.5Dの遠視であった。角膜径は両眼とも10.5mmで，眼軸長は右17.2mm，左16.8mmの小眼球であった。7年前に水晶体亜脱臼による閉塞隅角緑内障が左眼に発症した。レーザー虹彩切開術で隅角閉塞は解除された。2年前に白内障手術と眼内レンズ挿入を行った。11か月前に眼圧が上昇し，ドルゾラミド点眼で眼圧が正常化した。全周に虹彩前癒着があったが前房は深かった。7か月前にラタノプロスト点眼に変更した。4か月前に膨隆虹彩による急性閉塞緑内障が発症した。レーザー虹彩切開とドルゾラミド点眼に切り替えて眼圧は安定化した。全周に隅角閉塞のある小眼球では，ラタノプロスト点眼によって急性閉塞緑内障を誘発する可能性がある。
A 62-year-old female was seen by us 11 years before. Corrected visual acuity was light perception right and 0.3 left. The left eye showed hyperopia of+7.5 diopters. Corneal diameter was 10.5mm in either eye.The ocular axis length was 17.2mm right and 16.8mm left. Her left eye developed acute glaucoma following subluxation of the lens 7 years before which was controlled by laser iridotomy. The left eye underwent cataract surgery with intraocular lens implantation 2 years before. The left eye developed elevated IOP 11 months before which was controlled by instillation of dorzolamide. Three months after dorzolamide was replaced by topical latanoprost, acute glaucoma developed due to pupil block. Laser iridotomy followed by dorzolamide instillation brought the IOP under control. This case illustrates that topical latanoprost may induce acute glaucoma in a nanophthalmic eye with extensive angle closure.
