　63歳の男性の両眼に霧視が生じた。10か月前から，成人T細胞白血病，カリニ肺炎，サイトメガロウイルス（CMV）肺炎に対して加療中であった。左眼の前眼部に炎症細胞，両眼の眼底に滲出斑とトマトケチャップ様の出血があり，CMV網膜炎と診断してガンシクロビル点滴投与を開始した。その後一時的に抗原値が低下して眼底所見が軽快したが，抗原値が再度上昇し，滲出斑と出血が増加した。以後のホスカルネット投与は持続的効果を示さなかった。本症例は多剤耐性のCMVによる感染であると考えられた。

A 63-year-old man noted blurring of vision in both eyes. He had been under treatment for adult T- cell leukemia and pneumonia by pneumocystis carinii and cytomegalovirus (CMV) . His left eye showed inflammatory cells in the anterior chamber. Both eyes showed exudates and tomato ketchup-like hemorrhages in the fundus. These findings led to the diagnosis of CMV retinitis. Intravenous ganciclovir induced decreased level of CMV antigen and improved fundus findings. The CMV antigen level increased again followed by exacerba-tion of fundus findings. Substitution of ganciclovir by foscarnet failed to show lasting improvement. It appeared that CMV resistant to multiple drugs was involved in this patient.
