　70歳男性の両眼に，虹彩炎による続発開放隅角緑内障発作が過去8年間に反復していた。右眼にラタノプロストを1回点眼した。点眼後に虹彩炎が増悪し，点眼前28mmHgであった右眼眼圧は，2日後45mmHg,3日後55mmHgに上昇した。ステロイド薬の点眼，結膜下注射，経口投与で虹彩炎は軽快し，眼圧は22mmHg以下になった。活動性の虹彩炎を伴う続発緑内障がラタノプロスト点眼で急性に増悪することがあるので，注意が必要であることを示す症例である。

A 70-year-old male had had recurrent attacks of bilateral open-angle glaucoma secondary to iritis during the past 8 years. A single instillation of latanoprost in his right eye was followed by exacerbation of iritis. The intraocular pressure (IOP) was 28mmHg before, 45mmHg 2 days and 55mmHg 3 days after instillation. Topical and systemic corticosteroid induced remission of iritis and reduction of IOP below 22mmHg. This case illustrates that topical latanoprost may induce acute exacerbation of glaucoma secondary to active iritis. Due precaution is advocated in the use of latanoprost in such cases.
