ラタノプロスト点眼1～4年後の眼圧と視野

Intraocular pressure and visual field after one to four years of latanoprost instillation

要約　目的：ラタノプロスト点眼を1～4年続けたときの眼圧と視野の報告。対象と方法：正常眼圧緑内障50例100眼を対象とした。男性19例，女性31例で，平均年齢は66.9歳である。ラタノプロストの新規投与は21例で，他の29例は他薬からの切り替えであった。眼圧はGoldmann圧平眼圧計，視野はHumphrey視野計で測定した。点眼開始前の平均眼圧は13.0±2.6mmHgであった。結果：点眼1年と2年後では眼圧は有意に下降した（p＜0.01）。3年後と4年後の眼圧は点眼開始前と有意差がなかった。視野のMD値は点眼開始から1，2，3，4年後のいずれの時点でも，点眼前と有意差がなかった。結論：正常眼圧緑内障に対するラタノプロスト点眼で，点眼開始から2年間は眼圧下降効果があり，以後2年間は点眼前の値に戻る傾向があった。1～4年の点眼で，視野は点眼開始前と同じ状態に維持された。

Abstract.　Purpose：To report changes in intraocular pressure（IOP）and visual field following instillation of latanoprost for 1 to 4 years. Cases and Method：This study was made on 100 eyes of 50 patients with normal-tension glaucoma. The series comprised 19 males and 31 females. The age averaged 66.9 years. Latanoprost was used as the initial medication in 21 cases and as replacement of other drug in 29 eyes. IOP was measured by applanation tonometry. Visual field was measured by Humphrey perimeter. IOP before use of latanoprost averaged 13.0±2.6mmHg. Results：IOP decreased significantly after 1 and 2 years of latanoprost instillation（p＜0.01）. It remained the same as baseline value after 3 and 4 years of instillation. MD value of visual field remained the same after 1 to 4 years of instillation. Conclusion：Instillation of latanoprost resulted in decreased IOP after 1 and 2 years. There was a tendency for IOP to return to the baseline thereafter. Visual field remained the same during maximally 4 years of latanoprost instillation in normal-tension glaucoma.
