小眼球における閉塞隅角緑内障手術

Surgical and laser treatment for a case of nanophthalmos with angle-closure glaucoma

　Nanophthalmosが強く疑われる症例に合併した難治性閉塞隅角緑内障の1例を経験した．一連の手術・レーザー治療を用いることにより隅角と眼圧の安定したコントロールを得ることができた．手術はまず，渦静脈減圧術を施行し，corevitrectomyによる硝子体圧低下を確認してから，Kelman phacoemulsificationを行った．効率不良のために嚢外摘出術への変更を必要とした．水晶体摘出後に隅角癒着解離術とこれに続くゴニオプラスティーにより眼圧のコントロール・隅角の開放を認めた．しかし，隅角の安定はYAGレーザーを用いた後嚢切開術により，虹彩—後嚢癒着による瞳孔ブロックを解除してようやく得られた．

A 58-year-old female presented with nanophthal-mos complicated by chronic angle-closure glau-coma. Her left eye was phthisic due to total retinal detachment which developed followiig surgery for hypermature cataract 2 years ago. To relieve her right, only seeing eye, we initially performed vortex vein decompression and core vitrectomy followed by goniosynechialysis and extracapsular cataractextraction. Postoperatively, argon iridoplasty and posterior capsulotomy with YAG laser were neces-sary to correct the plateau iris configuration of the peripheral iris and to relieve the recurrent pupillary block due to iridocapsular synechia. The treatment was a success and the patient was discharged with normalized intraocular pressure and a corrected vision of 20/100.
