中心視野が消失しつつある最末期緑内障における線維柱帯切除術

Trabeculectomy for end-stage glaucoma with threatened central visual field

　固視点にほとんど達する視野障害と視力低下があり，視野が消失する寸前にある最末期緑内障4症例4眼に，マイトマイシンCを併用した線維柱帯切除術を行い，7〜14か月間の経過を評価した。全4眼で良好な眼圧コントロールが得られた。視力は1眼で大幅に低下し，3眼で維持または改善された。視力低下眼では，術前のfoveal thresholdが9dBで他の3眼での19〜33dBよりも著しく低く，術翌日の眼圧が2mmHgで他の3眼での12〜20mmHgよりも著しく低かった。

We performed trabeculectomy with adjunctive mitomycin C in 4 eyes of 4 patients with end-stage glaucoma. All the eyes had decreased visual acuity with the visual field damage almost involving the fixation point. The intraocular pressure (IOP) was well controlled in all the 4 eyes after follow-up for 7 to 14 months.Visual acuity either improved or remained stationary in 3 eyes and was almost lost in one eye. The eye with visual loss showed low foveal threshold value of 9 dB in conrast to 19 dB-33 dB in the other 3 eyes. It also showed IOP of 2mmHg on day 2 of surgery in contrast to 12mmHg-20mmHg in the other 3 eyes.
