放射線治療が著効した白血病視神経浸潤の造影所見

Angiographic findings in a case of leukemic optic nerve infiltration cured by radiotherapy

　33歳男性が両眼の視力低下で受診した。8か月前に慢性骨髄性白血病と診断され加療中であった。矯正視力は左右とも0.09であった。白血病に対する薬物治療により，6週後に視力は回復した。さらに5か月後に左眼視力が0.1に低下した。左眼眼底に，顕著な乳頭腫脹，乳頭周囲の隆起性病変，漿液性網膜剥離があった。白血病の視神経浸潤と診断した。総量46Gyの放射線照射で眼底病変は寛解した。フルオレセイン蛍光造影で，乳頭周囲にあった毛細血管拡張は顕著に改善していた。インドシアニングリーン蛍光造影では，照射終了後も乳頭周囲の隆起性病巣に一致した部位が低蛍光を呈していた。この所見は，白血病細胞の乳頭周囲脈絡膜への浸潤を示すと解釈された。

A 33-year-old male presented with impaired vision in both eyes of recent onset. He had been under treatment for chronic myelogenous leukemia for the past 8 months. His corrected visual acuity was 0.09 in either eye. It recovered 6 weeks later after intensified systemic treatments. Another 5 months later, his left visual acuity decreased to 0.1. Funduscopy showed swollen optic disc with peripapillary elevation and serous retinal detachment. He was diagnosed as leukemic optic nerve infiltration in the left eye. Radiation totalling 46Gy induced regression of the fundus lesions. Fluorescein angiography showed regression of engorged disc vessels. Indocyanine green angiography showed hypofluorescence in the area of peripapillary elevation. This finding seemed to show invasion of leukemic cells in the peripapillary choroid.
