Abstract. Purpose:To report a case of idiopathic orbital inflammation associated with presumed posterior ischemic optic neuropathy. Case:A 71-year-old male developed hyperemia and edema in his left eye and was suspected of orbital phlegmon. When seen by us 12 days later, the left eye showed proptosis, restricted motility, and relative afferent papillary defect. The corrected visual acuity was 2.0 right and 0.01 left. Magnetic resonance imaging(MRI)showed swelling of inferior rectus muscle. Systemic prednisolone was followed by visual acuity of 0.7 5 days and 1.0 7 days later. Inferior horizontal hemianopia was detected in the left eye 2 weeks after his initial visit. The visual defect persisted after recovery of visual acuity and persisted for one year until present. Conclusion:Posterior ischemic optic neuropathy may have developed after idiopathic orbital inflammation in the present case after ruling out possibilities of other systemic disorders.
