インドシアニングリーンを使用した内境界膜切除術とその術後経過

Outcome of removal of internal limiting membrane with adjunct use of indocyanine green

　特発性黄斑円孔，糖尿病黄斑浮腫に対し，インドシアニングリーン染色を用いて内境界膜切除術を施行し，その術後経過について検討したので報告する。症例は，特発性黄斑円孔10例11眼，糖尿病黄斑浮腫3例3眼であり，年齢は57歳から71歳（平均65歳）であった。インドシアニングリーン染色を用いることにより手術操作は容易になった。術後，走査型レーザー検眼鏡（レチナアンギオグラフ，Heidelberg社）を用いて眼底を観察すると，インドシアニングリーンの残存と考えられる蛍光が，全症例で術後1か月以上認められた。網膜嚢胞内，黄斑円孔底部にもインドシアニングリーンが残存する所見も観察された。今後も長期的な経過観察，検討が必要であると考えられた。

We performed surgical removal of internal limiting membrane (ILM) with adjunct use of indocyanine green(ICG) in 14 eyes of 13 patients. The series comprised idiopathic macular hole 11 eyes and diabetic macular edema 3 eyes.The ages ranged from 57 to 71 years, average 65 years. Intraoperative staining of ILM by ICG contributed to the ease andsafety of surgery. By observing the fundus with a scanning laser ophthalmoscope (Retina Angiograph, Heidelberg) ,fluorescence from ICG could be detected in all the 14 eyes up to 1 month after surgery. Fluorescence also persisted inretinal cysts and in the closed macular hole. This feature would necessitate a long-term observation of retinal functions following this type of surgery.
