　網膜色素上皮剥離（retinal pigment epithelium detachment：PED）を主病変とする加齢黄斑変性31例31眼を光干渉断層計（Optical coherence tomography：OCT）で観察した。OCT上，PEDは網膜色素上皮（retinal pigment epithelium：RPE）による高反射層のドーム状隆起としてみられ，RPE下腔は低反射域を示した。31眼中20眼で，脈絡膜新生血管（choroidal neovascularization：CNV）の部位では隆起したRPE層がくびれて「空間的notch sign」を示し，PEDの隆起は二峰性のドーム状に観察された。31眼中11眼で，CNVに一致して，剥離したRPE後面には高反射帯が接していた。この高反射帯の後方では脈絡膜の反射は減衰し，暗くなっていた。PEDを伴った加齢黄斑変性では，Gassの示した平面的なnotch signだけでなく，OCTでは三次元的にも「空間的notch sign」が存在し，これはCNVによりRPEとブルッフ膜とが癒着している像と考えられた。断層像でみられる［空間的notchsign」はCNVを示す所見として診断価値があると結論された。

We examined age-related macular degeneration using an optical coherence tomography (OCT). Retinal pigment epithelial detachment (PED) was identified in 31 eyes. PED appeared as a dome-like elevation of highly reflective layer corresponding to the retinal pigment epithelium (RPE) and the choriocapillaris, with an optical clear space beneath the PRE. The PRE showed constriction, or a spatial notch sign, at the site of choroidal neovascularization (CNV) in 20 eyes. A high-reflective thick layer was present posterior and in ap-position to the detached RPE at the site of CNV in 11 eyes. The findings show that PED in age-related macular degeneration may manifest a spatial notch sign in addition to the classical one described by Gass. It appears that the spatial notch sign as seen by OCT is due to the presence of CNV adherent to the RPE and the choriocapillaris, and that it therefore suggests the presence of CNV.
