網膜色素上皮裂孔の光干渉断層計所見

Optical coherence tomographic features of retinal pigment epithelial tear

（B2-2-15）　網膜色素上皮裂孔の4症例を光干渉断層計optical coherence tomography （OCT）で観察した。発病早期には．網膜色素上皮の裂孔縁には網膜色素上皮による高反射層の断裂があり，色素上皮の欠損部は網膜色素上皮による高反射層が欠損し，その後方の脈絡膜反射は増強していた。また，色素上皮が翻転した弁の部では極めて高い反射があり，その後方の脈絡膜反射は消失していた。発病晩期には網膜色素上皮裂孔は瘢痕化したが，色素上皮の裂孔縁および弁の部は，健常な色素上皮の高反射層と同じ高反射を示して境界不鮮明であった。色素上皮の欠損していた部位は色素上皮による高反射はなく，脈絡膜反射が増強していた。網膜色素上皮裂孔による網膜色素上皮の病的変化がOCTにより断面像として明瞭に観察された。

We evaluated the cross-sectional image in 4 eyes of retinal pigment epithelial tear using an optical coherence tomography (OCT). During the early stage, the tear appeared as disruption of high-reflective layer corresponding to the retinal pigment epithelium (RPE) . The flap of the tear appeared as a thick high-reflective layer with absent backscatter. The defect in RPE appeared as absence of high-reflective layer corresponding to RPE with enhanced choroidal reflection appearing as intense backscatter due to increase in optical transparency. In the chronic stage, the border of high-reflective area corresponding to the edge of tear became less distinct.
