We followed up 15 eyes of rhegmatogenous retinal detachment after apparently successful transscleral surgery using an optical coherence tomography (OCT). At 1 month after surgery, residual retinal detachment was present in 7 eyes (47%). It was localized in the macular area in 4 eyes and extended to involve the macula and the peripheral retina in 3 eyes. Its incidence was not related to intraoperative air injection into the vitreous.The residual retinal detachment gradually disappeared up to 12 months after surgery in all the 7 eyes except one. At 1 month after surgery, residual retinal detachment was present in 3 of 10 eyes (30%) with posterior vitreous detachment (PVD) prior to surgery. It was present in 4 of 5 eyes (80%) without PVD prior to surgery.The findings show that residual retinal detachment in the macular area is a frequent occurrence following appar-ently successful surgery when examined by OCT. Residual retinal detachment is more frequent in eyes without PVD. It is also a major factor for poor postoperative visual acuity.
