（18-レセ2-16）　原発閉塞隅角緑内障眼に対する手術治療後に生じた悪性緑内障3例の臨床像とその対処をまとめた。全例で術前にレーザー虹彩切開術により瞳孔ブロックは解除されていた。きっかけとなった手術治療は，2例においてPEA＋IOL＋トラベクロトミー＋隅角癒着解離術＋虹彩切除術の同時手術，1例でPEA＋IOLであった。YAGレーザー後嚢切開術および前部硝子体切開術を施行したが，器質的隅角癒着や生理的房水流出路障害のために，さらに手術治療手段が追加・併用された。

We observed three eyes which developed malignant glaucoma following surgery for chronic primary angle closure glaucoma. All the eyes had received laser iridotomy to relieve pupillary block prior to surgery comprising phacoemulsification and aspiration with intraocular lens implantation. Two eyes were treated by additional trabeculotomy, goniosynechialysis and peripheral iridectomy. After onset of malignant glaucoma, all the eyes were treated by YAG laser capsulotomy and anterior hyaloidotomy. Additional surgical manipulations were necessary because of goniosynechia and possible obstruction of the physiological route of aqueous outflow.
