トラベクロトミーと白内障同時手術の術後成績

Results of combined trabeculotomy and cataract extraction

　投薬のみでは眼圧コントロール不良であった緑内障に併発した白内障眼55例60眼に対して，初回手術として施行されたトラベクロトミー・白内障同時手術の術後成績を，白内障術式ごとに比較検討した。無投薬および投薬を用いて最終受診時眼圧を21mmHg以下にコントロールできたのは60眼中56眼（93％）で，術式ごとでは超音波核乳化吸引術（PEA）施行群13眼中13眼（100％），計画的嚢外摘出術施行群36眼中32眼（89％），計画的嚢内摘出術施行群11眼中11眼（1OO％）であった。併用白内障手術としてPEAは併発症も少なく，緑内障・白内障同時手術に適していると考えられた。

We performed simultaneous trabeculotomy and cataract surgery in 60 eyes with cataract and medically uncontrollable glaucoma. Postsurgical control of intraocular pressure under 21 mmHg was achieved in 56 eyes, 93%, with or without medication. Rate of successful control of intraocular pressure was 100% in 13 eyes after phacoemulsification and aspiration, 89% in 36 eyes after planned extracapsular cataract extraction, and 100% in 11 eyes after planned intracapsular cataract extraction. Phacoemulsifica-tion and aspiration is recommended when planning a simultaneous surgery for cataract and glaucoma.
