　水晶体後嚢が自然断裂し核が硝子体腔へ落下した1例を経験した。症例は68歳男性で，4年前から左眼の網膜静脈分枝閉塞症のため硝子体出血を繰り返し，白内障も進行したため手術目的で当科を紹介された。初診時，左眼視力は20cm指数弁であり成熟白内障のため眼底は透見不能であった。超音波検査では硝子体腔に異常を認めなかった。1か月後の細隙灯顕微鏡検査で後嚢の断裂と核の消失が疑われ，超音波検査で硝子体腔に可動性のある陰影がみられた．経毛様体扁平部から残存した水晶体を切除し，混濁した前部硝子体を切除したところ，硝子体腔に落下している核を確認し，これを除去した。今回の症例は白内障の進行に伴い後嚢の断裂を生じて核が落下したものと推測された。

A 68-year-old male was referred to us for cataract surgery. He had developed repeated vitreous hemorrhage as complication of branch retinal vein occlusion in the left eye since 4 years before. When seen by us, the left visual acuity was counting fingers at 20 cm for mature cataract. Ultrasonography showed normal findings in the vitreous cavity. When seen one month later, the posterior capsule had ruptured with loss of lens nucleus. Ultrasonography showed a mobile mass in the vitreous cavity. Pars plana lensectomy and anterior vitrectomy led to the detection of lens nucleus which had dislocated into the vitreous. It appeared that a spontaneous break of the posterior capsule led to dislocation of the lens nucleus.
