We reviewed the presenting complaints and findings by scanning laser ophthalmoscope (SLO) in 30 patients with unilateral macular hole. The series comprised 7 males and 23 females. The ages ranged averaged 67.6 ± 6.8 years. The macular hole was classified as stage 2 in 9 eyes, stage 3 in 12 and stage 4 in 9 after Gass. Metamorphopsia was the most frequent complaint in patients with stage 2 and 3 macular holes. Microperimetry by SLO showed a dense scotoma inside the macular hole in all the eyes. Few patients were conscious of central scotoma apparently due to the unstable fixation. SLO studies using a dark-field mode showed two types in the structure of the retina around the hole : fold type and dome type. Metamorphopsia was more frequent in eyes with fold type than dome type. The findings show that the subjective complaints are associated with the retinal structural changes around the macular hole.
