糖尿病網膜症に伴う黄斑円孔

Macular hole in eyes with diabetic retinopathy

　糖尿病網膜症に伴う黄斑円孔9例10眼（男性5例，女性4例）の臨床像を検討し，その発生機序を考察した。9眼は増殖糖尿病網膜症，1眼は単純糖尿病網膜症であった。円孔径は約1／5から1／3乳頭径大で，9眼では正円形を，1眼では楕円形を呈していた。単純糖尿病網膜症の1眼を除き，全例の黄斑部に網膜前増殖膜が観察され，肥厚した網膜前増殖膜が黄斑部を牽引している症例もみられた。10眼全例で黄斑部上の硝子体は未剥離であった。網膜前増殖膜による黄斑部の牽引が，糖尿病網膜症に伴う黄斑円孔の発生に関与している可能性が示唆された。

Macular hole was observed in 10 eyes of diabetic retinopathy. The retinopathy was proliferative in 9 eyes and nonproliferative in 1. The macular hole was 1/5 to 1/3 disc diameter in size. Its edge was smooth in all the eyes. Its shape was round in 9 eyes and oval in 1. Preretinal proliferative membrane was present in the macula in 9 eyes. The posterior vitreous was attached to the macula in all the eyes. The preretinal proliferative membrane appeared to be involved in the pathogenesis of this disorder.
