We reviewed a series of 14 eyes which received surgery for nuclear cataract after previous successful viterctomy for idiopathic macular hole. The best visual acuities, expressed as LogMAR values, averaged 0.93±0.25 before and 0.33±0.19 after vitrectomy, and 0.61±0.17 just before and 0.19±0.17 after cataract surgery. The differences among these four values were significant. There were no intra-or postoperative complications except reopening of macular hole after cataract surgery in one eye. The findings show that vitrectomy for macular hole may be followed by progression of nuclear cataract as an adverse effect and that cataract surgery is indicated for such cases.
