　円錐角膜による急性水症3眼に対し，前房内に空気を注入することによって，浮腫を早期に軽快させ，ハードコンタクトレンズによる早期の視力回復を得ることができた。急性水症によるデスメ膜断裂はほとんどが角膜中央部であるため，仰臥位である限り0.1mlの空気のみで十分なタンポナーデ効果が可能であった。空気が吸収され，タンポナーデ効果が不十分と判断したときにのみ，初回穿刺部から追加投与を行った。前房内空気注入療法では，浮腫消退までの期間およびハードコンタクトレンズを再開するまでの期間は，従来の保存的治療法に較べ有意に短縮された。この術式では，眼圧上昇，気体の後房迷入などの合併症はみられず，急性水症の治療法として積極的に試みられてよい術式と思われた。

We treated three eyes with acute hydrops secondary to keratoconus by injecting 0.1 ml of filtered room air into the anterior chamber. This procedure was repeated once in two eyes and twice in one eye. The three eyes attained clinical cure 9.3±1.2 days after air injection. Hard contact lens could be worn in 34.8± 6.1 days. There were no complications of the procedure such as ocular hypertension or air immigration into the posterior chamber. Another series of similarly affected 36 eyes were treated by conservative means. These eyes attained cure of the acute lesion in 63.8±30.9 days. Hard contact lens could be worn in 121.0±71.8 days. The difference between the two groups was significant (p＜0.01, two sample Wilcoxon test).
