　1980〜1991年の間に全層角膜移植術と白内障手術を同時または2段階で施行した47症例（同時手術19眼，2段階手術28眼）を術式別に，透明性維持率，視力成績，合併症について検討し，2段階手術における移植角膜内皮細胞密度についても検討を加えた。透明性維持率は，同時手術は19眼中11眼58％，2段階手術は28眼中17眼61％であった。視力改善率は，同時手術は19眼中14眼74％，2段階手術は28眼中20眼71％であった。2段階手術における角膜内皮細胞数は白内障手術前に平均1004.2cell／mm²であり，術後3か月に30.1％，6か月に46.7％の細胞減少を認めた。移植片への侵襲を考えると2段階手術より同時手術が望ましいと思われる。

We reviewed 47 eyes that underwent penetrating keratoplasty and cataract surgery during the fore-going 11 years. Both surgeries were performed simultaneously in 19 eyes and in two different sessions in 28. The transplanted cornea remainedclear in 11 eyes (60%) after simultaneous surgery and in 17 (61%) after two-step procedure. Visual acuity improved in 14 eyes (74%) after simultane-ous surgery and in 20 (71%) after two-step proce-dure. In eyes treated by two-step procedure, the endothelial loss averaged 30.1% at 3 months and 46.7% at 6 months. The findings indicate that simultaneous surgery is preferable to two-step procedure regarding damage to the donor cornea.
