We reviewed 47 eyes that underwent penetrating keratoplasty and cataract surgery during the fore-going 11 years. Both surgeries were performed simultaneously in 19 eyes and in two different sessions in 28. The transplanted cornea remainedclear in 11 eyes (60%) after simultaneous surgery and in 17 (61%) after two-step procedure. Visual acuity improved in 14 eyes (74%) after simultane-ous surgery and in 20 (71%) after two-step proce-dure. In eyes treated by two-step procedure, the endothelial loss averaged 30.1% at 3 months and 46.7% at 6 months. The findings indicate that simultaneous surgery is preferable to two-step procedure regarding damage to the donor cornea.
