全摘出による人工水晶体挿入における前房内空気による角膜内皮保護

Protection of corneal endothelium with air in the anterior chamber during cataract and lens implantation surgery

緒　言

　老人性白内障に対する全摘出法（ICCE，Intra-capsular cataract extraction）における人工水晶体（IOL，Intraocular lens）挿入の各段階において，前房内に大きな気泡を作りできるだけ角膜内皮との接触をさけながらIOLを挿入すると，術翌日より流涙，線状角膜炎の発生が少なく術眼を開放することがでぎ，明らかに従来の結果と異なる所見が得られた。

　この事実はMaumenee＆Stark^1〜3），　Drews^4）の普通のICCEとICCE＋IOLにおける角膜内皮細胞生体観察（Specular microscope）による比較でその角膜内皮細胞脱落に著明な差がみられなかつた報告によつても裏づけられる。これらの報告は何れも角膜内皮との接触を注意深く避けることにより角膜内皮は保護されたと報じている。

Loss of corneal endothelial cells constitute one of the serious complications in intraocular lens im-plantation surgery. Deep anterior chamber made by injected air and Ringer solution served as a defense against rubbing of the corneal endothelium by the iris and the intraocular lens during surgical steps of cataract extraction and lens implantation.

The current model of Hinkhorst 4-loop lens withits 10-degree posterior orientation of the loop made the dislocation less of a danger than the earlier type.It therefore dispenses with the transiridectomy suture and iris fixation at 12 o'clock.
