Prescription of hard contact lens may be difficult for keratoconus as the corneal radius decreases in the apex and the peripheral cornea becomes flatter along with progression of the lesion. We designed a new program for prescribing hard contact lens based on corneal topographic analysis. This program was applied to 9 eyes of keratoconus. The corrected visual acuity improved in 7 eyes after one month's wear. Continuous wear for 10 hours became also possible in these eyes. All the cases reported improved subjective feeling of contact lens wear. Severe complications were absent throughout. This program promises to be useful in prescribing hard contact lens for cases of advanced keratoconus that cannot be treated by ordinary hard contact lenses.
