Abstract Purpose:To report a case who developed persistent visual impairment due to corneal ulcer after wearing Miracle lens® for orthokeratology.

Case:A 24-year-old male was referred to us for ocular pain and impaired vision in the left eye since 2 days before. He had been wearing Miracle lens® for orthokeratology since 10 years before.

Findings and Clinical Course:Corrected visual acuity was 1.2 right and hand motion left. The left eye showed signs of bacterial corneal ulcer. He received treatments with topical and systemic antibiotics, and systemic corticosteroid. Inflammation in the left eye subsided 2 weeks later leaving subepithelial corneal opacity. Corrected visual acuity has been 0.3 left.

Conclusion:The present case shows that prolonged wear of contact lens for the purpose of orthokeratology may induce corneal ulcer with persistent visual impairment.
