Abstract Purpose:To report a case who received penetrating keratoplasty(PKP)for band-shaped keratopathy, who developed functional failure of implanted cornea with secondary glaucoma, and who received simultaneous PKP with implantation of Baerveldt® glaucoma device.

Case:A 73-year-old female presented with impaired vision in the right eye. She had received retinal detachment surgery 2 years before followed by silicone oil implantations and intraocular lens insertion.

Findings and Clinical Course:The right eye showed band-shaped keratopathy with visual acuity of 0.01. PKP was performed 8 months later followed by recovery of vision to 0.2. Intraocular pressure(IOP)elevated to 35 mmHg 2 years later with bullous keratopathy and decrease of visual acuity to hand motion. The right eye received simultaneous PKP and implantation of Baerveldt® glaucoma device. Twelve months later, the cornea remained transparent, IOP was kept below 10 mmHg, and visual acuity was 0.15.

Conclusion:Simultaneous PKP with implantation of Baerveldt® glaucoma device was effective for bullous keratopathy and glaucoma following initial PKP.
