弱視の予後判定に関する研究

A Study on the Determination of Prognosis of Amblyopia

緒言および目的

　「弱視とは，器質的変化がないか，あつてもそれでは説明のできない視力低下を認めるもの」とのBangerterによる定義が広く受け入れられているが，実際弱視治療にあたつてみると，器質的変化があると疑つていても，従来の検査では証明できなかつたり，あるいは器質的変化の量的測定が不可能なために，それらがどれくらい視力の低下に関与しているかがつかめないので，一括して弱視の名のもとに治療が行なわれてきた。そのために治る弱視と治らない弱視は，一応治療をしてみなければわからないということになる。

　著者は，弱視が本来は機能的疾患として取り扱われているにかかわらず，乳幼児の能力のために，従来の検査では見出すことができない器質的要素が，この中に含まれているために混乱を惹起したと考えた。弱視の予後判定には，弱視に交じつている器質的要素をいかにして見出すかにあると考え，次の3つの実験を行なつた。

Purpose : Before starting the treatment of amblyopia, organic diseases should be excluded. Detection of these organic diseases is thereforeconnected with prognosis. While no difficulty is expected in cases in which conventional oph-thalmoscope and slit lamp microscope revealed positive changes, no adequate method is avail-able for children for the assessment of the proportion of the functional part, and evaluation of the participation of the optic nerve diseases, causing no change in the eyeground.
