水晶体嚢真性落屑を伴う閉塞隅角緑内障の3例

Angle closure glaucoma in three cases of true exfolication of the lens capsule

　水晶体嚢真性落屑は，偽落屑とは異なり緑内障を合併することは稀であるとされているが，筆者らは高齢の女性の閉塞隅角緑内障眼に水晶体嚢真性落屑をみた症例を3例経験した。水晶体嚢真性落屑が比較的高齢者に多くみられること，特に熱線照射や外傷などの既往のない特発性の症例では高齢者に偏在することから，水晶体嚢真性落屑の発生に何らかの加齢性変化が加わっているものと考えられる。また，基底膜異常としての水晶体嚢真性落屑眼では，水晶体容量の増加や水晶体実質の代謝障害を起こしやすくなることが想定され，狭隅角ないしは閉塞隅角緑内障へ進展する一要因になりうるのではないかと考える。

We observed angle closure glaucoma in three females aged 71, 82 and 84 years respectively. All the cases showed true exfoliation of the lens capsule in the affected eyes. It appeared that the aging process of the lens was one of the factors for exfoliation of the lens capsule. The shallow anterior chamber appeared to be the consequence of metabolic disorders of the lens with increased volume, resulting in angle closure glaucoma.
